Nurse - Cardiology Care Coordinator

Provides Team Based Primary Care and other specialty services in VISN across the Enterprise, utilizing existing and emerging technologies. Uses technologies such as video conferencing, patients can be seen in remote areas throughout the nation by providers who are associated with VA Medical Centers. Virtual RN services utilize care/case management principles to coordinate care and provide patient management and support using health informatics. Telehealth technologies facilitate access to care and improve the health of Veterans with the specific intent of providing the right care, in the right place, at the right time. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. Clinical programs and specialties utilize these telehealth modalities and their technologies, in conjunction with specific clinical, business and technical processes to coordinate and deliver healthcare interventions. Such programs are developed and managed in accordance with established VHA and VISN directives and policies and these programs are critical to the overall Veterans Administration Mission to provide improved access to health care for Veterans. Required to provide critical medical, technical and program support necessary to sustain efficient telehealth operations for the VISN CRH. Responsible to facilitate communication and collaboration between all members of the healthcare team. Operational support coordinated and provided by this position includes direct patient care and the support of the delivery of health care through telehealth and face-to-face modalities. Responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. Demonstrates leadership by serving as an advocate for patients and collaborating with colleagues as she/he continues to enhance his/her own and the team's professional growth, development and practice. Minimizes fragmentation of healthcare delivery and optimizes quality of patient and family outcomes in a cost-effective manner. Supports the VISN CRH under the guidance of the Veterans Health Administration (VHA) which provides health care services to Veteran patients across the continuum of care in both institutional and non-institutional settings. Supports the location to and from which care is delivered, including but not limited to VA clinical locations and provider alternate worksites. Supports the VISN CRH which may provide Telehealth and face-to-face services not only to clinical areas within the VISN CRH. Provides services via multiple modalities, including virtual and in-person care. Collaborates with other members of the expanded health care team, facilitates coordination and transition of care between services, and may involve the patient and family in the decision-making process. The incumbent works effectively with all levels of staff, to include collaborative efforts with other health professionals, providing ongoing care management/coordination services. Provides coverage for other VISN 16 CRH RNs when they are unavailable to perform these duties. This VISN 16 CRH RN position is organizationally located in the VISN or facility CRH line and attached to Central Arkansas Veterans Healthcare facility. The department description and patient population served is defined in applicable Scope of Service. Due to the virtual nature of the work and the provision of care using technology, the CRH RN may telework and/or be within reasonable proximity of a VISN 16 facility. Responsible for managing care across the health care continuum for a select group of clients. Responsible and accountable for all elements of the nursing process when providing direct patient care. Independently provides professional assessment, coordination, telephone triage and planning of multiple health care services; acts on behalf of the Veteran to assure that necessary clinical services are received, and that progress is being made. Responsible for providing direct patient care to Veterans and training and support to CRH and Spoke site staff as appropriate. This position requires flexibility in schedule and assignments, and occasional to frequent travel across the VISN. Must be poised and articulate in communication and works cooperatively in a team environment. Makes caring for the Veteran his/her highest priority in care delivery and demonstrates customer service skills in all aspects of work. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The position may require long periods of sitting through meetings and/or at the computer. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking (2-4 hours), standing (2-6 hours), stooping, sitting, bending, pulling (1-2 hours), and pushing (1-2 hours). lifting (>50 pounds), carrying (>50 pounds). Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
